52 yr female with obstructive jaundice due to pancreatic head mass lesion (metastasis) with complete duodenal obstruction.

Patient had distal gastrectomy due to carcinoma stomach and gastro jejunostomy

Enteroscopy assisted ERCP failed.

Hence Endoscopic ultrasound guided trans gastric metal stent placed into left lobe bile duct leading to releif on jaundice

EUS procedure is better than PTBD and leads to internal drainage of bile
